I think that was the most bloodiest surgery I've seen Black Jack perform. That was also one heck of a thriller episode, mixed in with a murder mystery, detective work, and yet another medical case that utterly stumped the good doctor. As always, this OVA series has been great. It's by no means realistic, but it was never the intention. Or, rather, I enjoy how seriously it tries to make sense of unnatural, supernatural, and fantastical illnesses. Would realistic illnesses be more interesting? Of course, as it would ground the series in a relatable setting. I still recall previous episodes that dealt with politics, war, famine, and epidemics. Those were great, but I also respect the series for not even attempting to create implausible illnesses that fake being real. It instead goes all out with being as unrealistic in cases such as this episodes, so that it attempts to never be insincere about its portrayal of medicine. At least, that's what I got out of this.

And also, I've mentioned this before, but I love how theatrical this all feels. It's really something special.

Yeah this is probably why I really enjoyed this episode. I like that detective character.
Not the best detective story, that's sure, but, as you said, the cinematic style makes it really fascinating and believable.
Too bad 12 episodes are too few to really get attached to the characters.

I liked this case, a lot. But it went to far beyond a medical case. Actually it went simply too far beyond the eponymous manga chapter with this full transformation power.

Should have called for an exorcist, not doctor xD

I didn't come here for Supernatural anime like Mushishi
But I enjoyed it so far
Black Jack is a unique character, cool, calm, and a efficient doctor

Funny story, but this was the episode of anime that got me into anime. I was 11 years old and my family were visiting my dad's coworker's place for dinner, and they let my brother and I watch TV in the lounge while the adults ate and chatted amongst themselves. They had cable, which our family didn't have at home, so naturally I was really excited to check out some of the channels I didn't have, which included Animax. I immediately stumbled across an explicit scene of a guy on an operating table, followed shortly by him getting up and slashing a dude with a katana. Again, I was only 11 and hadn't seen anything that shocking before, especially not in any animated shows I'd seen. Naturally I was fascinated and here I am.
