Medical-drama manga created to criticize the Japanese medical society. They are quite cynical, and do not mind touching upon sensitive and taboo subjects to get the message across. The protagonist is designed to be a good and morally-just doctor who goes to extreme lengths for the sake of his patients and refuses to back down in the face of the (exaggeratedly) corrupt and immoral community of doctors around him. The main difference is that Kirihito Sanka has supernatural elements while BJnY is grounded in reality.

Both are medical manga dealing with doctors as main characters. Whereas Dr. Koto is more light-hearted and irrealistic (a doctor that can perform miracles), Say hello to Black Jack is of completely different vibe (the doctors are powerless and the medical world is corrupted by competition, money and apathy).
