It's not just the toothbrush scene, it's the entire episode, it literally feels like a porn setup.

Tsukihi is away, Koyomi and Karen are all alone in the house, Karen decides to wear clothes that are too small for her, Koyomi notices her in "that" way for the first time, and then they play toothbrush punishment game.

It literally feels like there is lost media sex scene somewhere out there, I want to believe.
