Do you think living in Japan instead of Korea will lead to substantial differences in how Elise thinks and goes about things compared to herself in the manhwa?

No, not particularly. Though it's been a long time since I read the manhwa/webtoon I am sure there won't be much difference at all, and if per chance there is the author would have made up for it with similar Japanese situation.

This story is a back(?) reincarnation, she goes back to her previous life [warning: ending spoilers which will not have too much effect but read at your own risk]

... If you think about this trope it is quite common in both manga and manhwa, nothing tooo much out of ordinary, personally having read so many reincarnation stories the thing which surprised me was the last part of the spoiler I gave.

Back on the main topic, the story mostly talks about medicines, a little war and stuff which, well the whole world knows so even if Elise was a Japanese it wouldn't have mattered I hope you got your answer?

In the manhwa, her name is Song Jihyeon. In the anime, it's Takamoto Aoi.

changing their Nationality has been around for some time now and now you guys are complaining about it?
well it does sounds reasonable but it's the writer of the anime chose to have it changed. i'm sure the author of the webtoon/manhwa would even know it first hand. before they adapted this into anime, the company had to buy the rights before adapting it and thus discussion about the nationality and name changes are also apploed to it as well.

i've read alot of reincarnation as well, mostly human to monster. they are popular in manga and manhwa.

@arinthach that is a good question indeed. let say that the writer already knew this situation and tried to change the mentality before and after the change.
yes she is a Korean, but iRL Japanese and Korean have something similar. we'll never know once they release more episode.
majority of the Korean drama when adapted to any Asian country have their nationality and names changed, its quite common here in Asia.

I remember watching a K-Drama that was a Chinese Historical Drama but the names and nationality switched from Chinese to Korean.

Not really. Her medical knowledge is all that matters. Although I want to stay as close to the original source as possible, her ethnicity is irrelevant.
