To build on the other answers- the hair pin is made of deer skin, and she says it's one of the most important/meaningful things she owns. Why, I don't know but I'm thinking it's a gift from someone from her far past

It's a Barrett, just a large ornamental hair clip.

SPOILER:

She wore for as long as the manga shows, the one she wears currently was gifted to her by the Emperor when she helped him win over Princess Kaguya's heart.

It is also shown, that Tsukasa's immortality affects the Barrett, since Tokiko's studies show that it posseses the same regeneration abilities as Tsukasa, and it can never be destroyed.

That is also confirmed by Tsukasa herself, in the newest chapters of the manga.
