Otouto no Otto is the perfect homophoic introspection story, there is no annoying sense of being overdramatic, it handles all of its subject matters carefully and it does it in a very concise manner. Both focus on the struggles of queer people in Japan. They aren't super similar besides that point, since Otouto no Otto doesn't focus on love based relationships like Ao no Flag, choosing instead to mostly focus on a man accepting his brother's husband.
