Overall theme of the manga and character motivations are very similar.
Both main characters are badass monster killers, very knowledgeable about the best ways to dispatch the nasties. The ways The Goblin Slayer sets traps and improvises during battle is very reminiscent of the early Guts.
Also all the gore and rape...
I would even risk saying that Berserk was the main inspiration to Goblin Slayer.

1.Both have a main character with a VERY strong motivation to kill the enemies.
2.Both have an mc who want to protect someone close to them while having the burning passion for revenge.
3.Both of them have a dark setting where there are a "few" character deaths and no mercy is shown at all.

I usually compare this work to Berserk in that it is brutal with the deaths and merciless with those who fall into the hands of the goblins. The point that differs from Berserk is that even with these brutalities, the work continues with a less heavy and happier (?) atmosphere, so to speak.
