Both anime focus on a doctor who, in order to operate his patients and save their lives, has to break the law. Both anime show the journey of these doctors, who, by meeting and operating new patients, also question their own sense of morality.
Apart from the story, both anime pay homage to the works of Osamu Tezuka: Young Black Jack is a prequel to Osamu Tezuka's Black Jack, while Monster pays homage in more subtle ways (the most notable being Dr. Tenma, who shares his name with a character from Osamu Tezuka's Astro Boy).

Both anime adaptations feature the struggles of doctors and how they are viewed by other people in society for their crimes and choices.
