Psychosis is basically just messed up in the head. It is often characterized by hallucinations (false sensory things) and delusions (false irrational beliefs). This usually means things like paranoia and anxiety. Psychotic people are not to be confused with psychopathic people, who simply cannot feel sympathy for anyone or anything. It seems to me like Johan is a little bit of both, probably due to his terrible upbringing, and I doubt a bullet straight to the middle of the brain really helped his case. His never got the opportunity to see the good in the world, so he developed a very delusional point of view.
I feel like most shows just kind of make a character act weird and crazy and then say he/she is psychotic without really going into much detail or making us sympathize with their lack of control. Monster did a REALLY good job of making it clear that Johan was not entirely at fault for his actions. His brain was just screwed up. Everything he did just seemed... off in one way or another. I felt bad for Johan while watching Monster, which means Naoki Urusawa must have been doing something right.
It is never actually confirmed that Johan has some sort of illness like schizophrenia, but I think psychosis is just the broad term for being psychotic and deranged whether it be from illness, trauma, or injury.
If you don't mind, what did you think of the quotations used throughout the piece? I thought it gave the article a sense of narrative and also provided context for many of the ideas Urawasa was trying to explore. I thought it was fitting that after the Talmud(spiritual), Darwin(scientific), and Tacticus(history), that we end the article with a quote from Johan himself and we also began it with the book of revelation quote like the show as well. I'm rambling a bit, but I love all kinds of feedback and I was worried the quotes were gonna fall flat.
