Well basically you have to be able to handle using CSS code as well having a semi-decent understanding of how Photoshop (or similar) works.

A good place to start is to have a look a Ranivus's blog entries:

Once you have the basics down it's not that hard to get a hang of. If you understand where to put your headers and background code you can then focus on making your own images.
