True Identity: Arturina Pendragon (King Arthur)
Based on the legend of King Arthur, except Uther Pendragon has a girl insted of a boy. Thinking that the citizens wouldn't rally behind a girl, Uther hid her gender from his citizens and she was given to Merlin. Soon afterwards Uther died with no apperant heirs and so began the quest for a new king by pulling the sword from the stone. After years of trying the citizens gave up that anyone could pull the sword, until one day Arturina had done the impossible and freed the sword and claimed her rightful place as king?

True Identity: Hercules
Berserker's noble phantasm comes from the legendary 12 labors of Hercules.
In a fit of madness, induced by Hera, Hercules slew his wife, Megara, and their three children. The fit then receded. Realizing what he had done, he isolated himself, going into the wilderness and living alone. He was found and convinced to visit the Oracle at Delphi to regain his honour. The Oracle told him that as a penance he would have to perform a series of ten tasks, or labours, set by King Eurystheus, the man who had taken Hercules' birthright and the man he hated the most. King Eurystheus tricked Hercules into doing twelve labours but originally it was supposed to be ten.

I guess the creators of the series gave Berserker 1 life for every labor the legendary hero accomplished.

True Identity: Hound of Culann (Cú Chulainn)
Cú Chulainn is an Irish mythological hero who appears in the stories of the Ulster Cycle, as well as in Scottish and Manx folklore. He gained his better-known name as a child after he killed Culann's fierce guard-dog in self-defence, and offered to take its place until a replacement could be reared. It was prophesied that his great deeds would give him everlasting fame, but that his life would be short. In battle he becomes an unrecognisable monster who knows neither friend nor foe.

True Identity: Medea
Medea was the daughter of King Aeëtes of Colchis, niece of Circe, granddaughter of the sun god Helios, and later wife to the hero Jason, with whom she had two children. Jason eventually left Medea for another king's daughter. Medea took her revenge by sending Jason's new love a dress and golden coronet, covered in poison. This resulted in the deaths of both the princess and her father, king Creon, when he went to save her. Medea continued her revenge, murdering her two sons by Jason. Afterward, she left Corinth and flew to Athens in a golden chariot driven by dragons sent by her grandfather Helios, god of the sun. Medea is known in most stories as an enchantress and is often depicted as being a priestess of the goddess Hecate or a witch.
Rider - Servant

In Greek mythology, Medusa "guardian, protectress" was a gorgon, a chthonic female monster; gazing upon her would turn onlookers to stone.
I was iffy about this choice for a servant as Medusa's gaze would have been a real good weapon against the other servants, but it was never used. Pegasus I could see, because in the actual myth when Perseus severed Medusa's head from her neck, two offspring sprang forth: the winged horse Pegasus and the giant Chrysaor (never used by Rider) who later became the hero wielding the golden sword.
Gilgamesh - Servant

The eighth "secret" servant final boss and king of all heroes, Gilgamesh of Babylon, the greatest king on earth, two-thirds god and one-third human, as the strongest King-God who ever existed. The people in the time of Gilgamesh, however, are not happy. They complain that he is too harsh and abuses his power by sleeping with women before their husbands do, the goddess of creation Aruru creates the mighty wild-man Enkidu. Enkidu starts bothering the shepherds and jostles the animals. A trapper complains to Gilgamesh, the king sends the woman Shamhat who was a temple prostitute. The body contact with Shamhat civilizes Enkidu, he is no longer a wild beast who lives with animals. Shamhat convinces Enkidu to come to Babylon and be a man of society. Meanwhile, Gilgamesh has some strange dreams; his mother explains them by telling him that a mighty friend will come to him. Enkidu and Shamhat leave the wilderness for Babylon to attend a wedding. When Gilgamesh comes to the party to sleep with the bride, he finds his way blocked by the mighty Enkidu who opposes Gilgamesh's ego, his treatment of women and the defamation of the sacred bonds of marriage Enkidu has come to honor. Enkidu and Gilgamesh fight each other After a mighty battle, Gilgamesh breaks off from the fight after defeating Enkidu, sparing his life yet also heeding what Enkidu has said. The event teaches Gilgamesh the virtues of mercy and humility, along with courage and nobility. Gilgamesh is transformed for the better through his friendship with Enkidu, and vice-versa, as they have many lessons to learn from each other. They even begin to see each other as brothers in time. Soon after Enkidu dies, leaving Gilgamesh with such a sadness that he went on a journey of self-discovery leaving his kingdom Babylon to wither away and die without his protection.
Archer - Servant

True Identity: Shirou (alternate/ future reality)
Knowing this made the anime ending for myself a lot better, knowing that Shirou became the hero he always wanted to be. " HINTS" The second opening shows both Archer and Shirou fighting in the Unlimited Blade Works and his opening was shown after Archer's demise at the hands of Berserker. Shirou's main ability was to make things out of his imagination, the Unlimited Blade Works was the limbo the future hero Shirou had gone to after he died and in the UBW he crafted sword after sword trying to make a sword as powerful or more powerful than Excaliber, thus all the swords laying around in the UBW.
