I watched this on Netflix rentals back in the mid 2000s, bought it on DVD shortly after and have never rewatched. I'm almost afraid to ruin my initial feeling by doing so.

I watched it for the first time about 2-3 years ago and I LOVED it. It definitely holds up, it’s one of the best dramas I’ve seen.

Maybe you shouldn't rewatch because it will ruin the initial feelings but you should if you want to. I really liked the anime when I watched it for first and never rewatched (I am a guy).

I watched this a grand total of 3 times at different stages of my life and every time I found something new to appreciate about this series and the characters.
I admit that the first two times I didn't really like or understand Yukari because I was still in the mindset that I have to relate to the main character. However, the third time around, watching it as just a story, without trying to relate to the characters, it is truly a beautifully crafted tale. Not saying that you can't relate to them but, as an adult, I find I relate to them differently than when I watched it at 17.
If back then I was disappointed by the ending, now I see that it was truly for the better.
All in all, I think that if you watched paradise kiss before 20-22 years old, rewatching it will give a different feeling. Not spoiling the first impression, but still different.

Truly, I think Ai Yazawa is one of the best drama writers. Nana and Paradise Kiss are complete masterpieces, but Paradise Kiss having an ending gives it a whole new edge. Besides being a pretty good ending, it leaves you with too many lessons. I would have liked to see it younger.
