the final physical age gap between both?

Anzu waits 8 years for him so she is 25. Kaoru's wish of wanting to see his sister is granted, so surely even his lifespan must be reduced, (I say this because of the starting dialogue in the movie: "Do you know about the tunnel that grants you whatever you want? What the heck? When you walk through the gate in it, you'll come across what you want. But you'll age 100 years in return")

Then, both of them spend time equivalent of 5 years until Kaoru again wakes up. Overall, they are mentally 30 years old, but how old are they physically? Anzu probably will still be 25, but how about Kaoru? Also, similar to how Kaoru's wish was granted, even Anzu's wish of getting back her manga pages was granted, so wouldn't she age as well from that?

8 years is the difference. And I don't think there's any penalty here. What you quoted was more about how the time flows differently inside of the cave compared to how it does outside of the cave.

probably a subbed mistranslation.

as the fellow watchers said above
it was just a myth while the real truth of the cave was different
its not uncommon for rumours or reality to be different
as for age girl is 25(physically and mentally)and 30 according to the world
and for guy he is 16-17 (physically and mentally) and 30 according to the world
Blah blah blah blah .... So annoying. Are you bastards in heat or something?

@ktg The penalty here is the time you lose outside. He basically lost 8 years of being with her love so he could be with his sister for a few hours. Imagine that you're a father of a new born and you many hours inside of the tunnel. You lose all the memories and fun time of watching your kid grow. You family could be dead when you get out. basically you lose "years of your life".

@nocivo That's not really a penalty, because it only applies to some people. Like in the anime, the 2 kids are willing to sacrifice anything until they met. So their "penalty" does not exist. And there are others on the planet, orphans who have nothing.
Penalty is only a penalty if it applies to anyone.

@ktg You're splitting hairs. If it pleases you then call it a "cost" instead of "penalty." Either way it's obvious what nocivo meant.

Again, it is still not "cost", because it does not apply to everyone. The best way to call it is side effect and based on people's situations it can be good and bad.

You're fundamentally misunderstanding the term.

Cost is neutral and doesn't have positive or negative connotations, unlike penalty. The cost of a trade may be something you wanted to part with, or it could be something that makes you hesitate. It's still a cost either way.

Think of end-of-year departmental budgets. If you have to spend a certain amount of money this year to qualify for the same budget next year, you will gladly pay the cost of whatever you are purchasing, because you are deliberately trying to spend that money. You didn't lose all that money as a "side effect" of the purchase, you paid it as the cost (or price) of the transaction.

It's like the concept of equivalent exchange. You have to give something to obtain something you want. Whatever it is you lose isn't merely a side effect.
