I don't think it's the most messed up show out there, in my opinion shows like Higurashi, School Days, Euphoria, are much more messed up. There are more but I can't recall any at the moment. The only one I can recommend is Higurashi, the rest is trash.
But I can totally agree that Happy Sugar Life is great, can totally recommend that too.

True, but Metamorphosis doesn't have an Anime adaptation at this point, so I don't count it.

while happy sugar life is definitely very messed up, the anime that made me the most uncomfortable was without a doubt Yosuga no sora. While there’s nothing in it as bad a euphoria, it’s honestly really disturbing. It goes through different scenarios where the mc hooks up with different girls from his harem. Several people walk in on each other, and it’s really messed up. I wouldn’t recommend it.

If you want some action involved, I recommend Kara no Kyoukai & Fate Series(especially Heaven’s Feel). It’s based on Type-Moon’s Visual Novel. And seriously, that company never make anything normal.

Lol tyy I just haven’t been watching the long series recently since I’m trying to shorten my plan to watch list. I’ve heard so much about Fate, and it seems interesting, I’ll definitely put them on my plan to watch though

Ohh also, Kara no Kyoukai reminds me of Shiki. Idk if you’ll like it, but I really liked it, especially the ending. The beginning of the first episode is dry though

Higurashi is way more messed up. If you're going to watch it, the watch order is: Higurashi (2006) > Higurashi Kai > Higurashi Gou > Higurashi Sotsu. In my opinion, Gou and Sotsu aren't great. Just watch the first 2.

I have to agree that this anime is One of the Most Disturbing/Mess up anime I watched.

The reason is, Some disturbing anime rely on body horror or Monsters eating/killing people to disturb their audience.

In Happy Sugar Life edge is, It can happen (or it already happen) in real life!

I feel bad for Shio. I may not have exactly the same experiences, but change in her behavior towards the end and the final scene in the hospital are what is probably the most interesting and horrible at the same time. If you ever end up being somehow abused and I mean all flavors of it, not only in the way she was, there are two most possible scenarios. Scenario one is that you get hooked on it, desperately try to forget and change, but it will be a mix of hatred towards whoever wronged you and craving for the pain at the same time. Scenario two, you will gravitate towards becoming someone like the person who abused you. Shio seems to be the type two since she's too small to realize how fucked up the whole thing was and she would most likely become an abuser. Personally, I'm the type one since it takes time to get through therapy and stop getting yourself exposed to more harm. This anime is fucked up, but damn it's real. Viewer would be brain dead not to realize that there are no positive characters (ok, maybe cops and the murdered friend weren't that bad). Despite that, I feel like there are more messed up productions, yet this one is in my opinion more likely not to show clearly the brain dead type of viewer, that it is not cutesy romance, but story about abuse fueling abuse and misery accompanying it

I feel like Redo of Healer was just edgy because it's fun to be overly edgy. I just finished that show before finishing Happy Sugar Life and they are definitely two different flavors.
I much preferred Happy Sugar Life because it felt more psychotic. In Redo, the MC knew full well what he was doing and orchestrated his plan from square one but Satou was simply trying to keep her balance in the situation she got herself into.

I'd put it up there as one of the darkest anime I've ever seen. The fact that it feels so real is what makes it such an interesting watch. It wasn't my favorite in terms of being an exciting watch but I enjoyed it anyways. I agree with a lot of the other recommendations. School Days and my favorite series: Higurashi(all of it) are right up there with this show because of how twisted they can be. School Days imparticular felt way too real and made me uncomfortable, which I really enjoy with these shows. School Live! Is also very dark and sad (made me tear up a few times).

I've talked to Shindo many times and he says that even a hentai adaptation won't ever happen because of the drug use. Drugs are extremely taboo in Japan, alot more than sexual abuse or incest. So he said it'll never happen.
