In the Chapter 1 Discussion I questioned whether the romantic feelings between the siblings was mutual or one sided. It appears that the feelings are in fact mutual and upon realizing that the girlfriend is indeed a substitute for herself, the sister is overwhelmed by an ever growing sense of loneliness.

It appears the brother intentionally distanced himself in order to protect her. Due to her weak heart, if he were to give in to his urges and physically express his love for her her heart could give out and she could die.
