Based off of the synopsis, I had suspected the reason the older brother is only able to get it up for a woman when she's asleep is because he had been raping his unconscious sister. We appear to get glimpses of that in this chapter, however, it is not completely clear yet whether those panels are dreams or repressed memories. We also don't yet know whether the siblings are in love or whether the love is only one sided.
