The two elves even look like her. I understand that there was a foster system but I kinda want them to be her kids and nod adopted (except the dwarf). Its rare in anime for the main character to actually have children.
I’m pretty sure she used the foster system to adopt all of the kids that she has, she explicitly says a couple times that she never dated and never married within the game, so I highly doubt she is would’ve had children.

You know, I have been wondering this too. The three older children were created as NPCs when Leadale was still a game, so it is entirely possible that the two elves she created were based off her own personality and looks. The dwarf not so much. I have only seen the anime, so I don’t know if the source material has more information on their backstory.
Although, as a biology buff, since this all happened in the game, the children are likely not biologically related. My bio brain is screaming “no sex means no babies that are different” (females of some animal species can reproduce on their own, but all offspring would be clones).
If we were to assume that the children (the elves anyway) are 100% biologically hers then she had to have had a partner in the game; however, she has never mentioned having one let alone dating anyone.

Mai-Mai and Skargo are her biological children, but she didn’t give birth to them since she created them with the foster system.
You can choose your relationship with the NPC when creating them, and she made Skargo and Mai-Mai as her biological children and Kartatz as an adopted son.
Well, depends on how you see it. she said that she created them herself, but she was not pregnant. They are NPCs that she created herself, but since it was not a game, they are not really biologically hers

When she meets by "chance" with a high-elf "Adventurer" and his "Sister", its revealed that he is actually a spy for a foreign land. One that High-Elves rule over, bordering the "Lost City". He works directly for the Queen (Sahalashade) of said land, a Noble High-elf whom was a NPC-child of the first High-Elf Player Sahana of Leadale. This person turns out to be close friends, and she writes in her player registration to be a sister to cayna. Leaving her "Daughter" to cayna to care for, as the toxicity around high-elf characters in leadale drove every high-elf to quit/switch characters (except cayna). While its never said directly, its hinted that Sahana was Opus alt, and cayna hints at having a soft side for opus, even including possibly including juxtaposition about opus in her childrens bio.

So its never mentioned that cayna had a partner, due to how the former game systems integrate with the reality that is leadale one of two options can be deduced. Either the children were "Magically Concieved" using acient magic, taking on cayna's traits (like we see already with their magical skills), or there was a proprietor via the characters bio's, that cayna herself doesnt realise due to how it was written, being taken as a "Joke".
Remember, Cayna barely accepts Leadale as "Real" and still sees alot of it as a "Game+" like she is still part of some bigger system. (more on that to come)
Getting to the other side of this, its important to note that while the anime/manga/ln/wn doesnt exclusively say she "Birthed" skargo/mai-mai, that may of been exactly what happened. The disconnect between how game systems create phenoma from just text says alot, as cayna doesnt regard passing words in the past as reality. Treating Leadale of the past and Leadale of Today as two seperate things when they are in-fact connected and everything has meaning, She is just not aware of it.
This is confirmed more so when
Little fairy is revealed to be the one making the former game systems (outside some magic) still work. Every advanced spell that relied on some former game system to function (think of her storage box or "Map"). However, its very light on exposition as its only directly hinted so far in the LN when she casts a tracking spell and it is revealed the fairy is the one putting out the juice

All this said though, events in the anime are not lining up with the LN many cases. Complete events are being rewritten to be much simpler where characters that should be involved to develop backstory are just missing/ignored and/or rushed. Strong supporting characters that should of been part of this season are missing. Latem, which should of _NEVER_ been cut as its a key setup to parts that were supposed to go along with the mermaid and first-flight that happened at the end of episode 12 are just cut all together.
Its important to note, even things like the bridge that Kartatz built, was supposed to be a joint venture with cayna. Not treated as a non-existant ploy...
Worse off still, is the complete absence of all the visits to Sakaiya, to set up much of what should be part of season two.

All that said, the anime doesnt really take into account Skargo/mai-mai's existence, amongst other player-characters npc's which she has already interacted with. Choosing to handwave them in a rush to get to the next action scene. This differs greatly to the LN
where it gives the characters life and character development. The anime doesnt even show /how/ they got their abilities or /why/ they are in the positions thay are in. Even Kartatz, which gets the most amount of screen time of the siblings that not just filler in the anime, has a crap ton more backstory that the anime is glossing over just in the first 4 episodes... The seasons are too short for the amount of story the anime has covered.
Im worried the animators are rushing through this to get the series over with/concluded given the 12 episode limit. as its more then 80% through the source material already, but didnt cover the hows, whys or set up the conclusions that will end the series next season.
This is concerning, and may leave questions such as these never answered by the anime as a result..
(EG: This is another Fullmetal Alchemist vs FMA:B, situation. There is no hope getting to the why's and hows, when the end is so near due to rewriting scenes to avoid character developments and introductions. The story is fundamentally changing to suit what ever ending they are going to give it.)
i think they were all part of the foster system, btw could someone explain how cayna started off in the inn in ep 1? it's sort of strange how the whole story didn't explain how she even got there after 200 yrs.

If it makes you feel any better, her status specifies that Mai Mai and Skargo are her blood related children, while Kartatz has "adopted" written in front of Son, in the episode in which he is intoduced.
