Well he shot him, and said he caught the culprit, so that's my guess :P
Does anyone else need a doctor to confirm that someone without a head is dead?

I think that they never caught that guy. And by that, konakawa had his problem and wanted to get over it. So he just saw a dream that he caught him and is satisfied with that. But because of that connection with dreams and reality he actually caught the culprit and shot him dead............is what i think.

The situation was a macguffin. Who killed him doesn't matter so much as the internal struggle facing the characters dealing with having lost the guy.

From what I understood, it didn't matter who the culptrit was. It was more of an internal struggle of Konakawa.

He died of natural illness. Konakawa had a trauma about him being killed because he felt that his friend died because of him.

I think Konakawa caught the perpetrator but it wasn't shown who it was. But it was mentioned in the last scene where Paprika mentons that Atsuko has last name shared with Tokita.

The homicide was a figment of Konakawa's imagination, a sort of metaphor for the survivor's guilt that Konakawa felt for not finishing the movie, and for his friend dying after being accepted to film school.

As others have said, it doesn't matter who did it, it's more that the case rekindled memories in the detective who had suppressed anguish over his friend's passing.

Contrary to the post above me, the homicide was real and Paprika congratulates Konakawa for catching the culprit in her email letter. The implication being, once the detective cleared his mind of his worries, he could focus on the case properly and do his job.
