He gets arrested after the dr finds him with a police presence following him. I suppose you could say the deeper meaning would be that he finally accepted his delusions and the dr finally accepted that he might be way more feminine than he liked to admit previously

Well it's up to you determine that, the ending is ambiguous on purpose.

Overall Nakoshi practically loses all touch with reality and is forced to give up his delusions at the end when he finally gets arrested. That is the obvious explanation of what happened, however how you interpret Nakoshi's perspective of what happened to him is up to you, the reader.

It's important that you come to understand Nakoshi's delusions based on your own life experiences and comprehension of Nakoshi's own emotions throughout the story to fully grasp the ending as well.

In the end instead of knowing what is it to be human he finds about what happens when he drops soap in jail
