I'm really confused about Makoto's genre. To be honest, I didn't realize it was breasts when Jin cut her clothes in episode 3. With episode 7, Makoto is mentioned with a "she" by Jin and appears as a demon with well marked breasts.

How to define her and how does she define herself? On Wikipedia, she's described as a man disguising himself as a woman (edit: my bad, the correct sentence is "described as a woman who disguising herself as a man"). I read a comment on her Fandom page that trans people were common in Japan at the time of the events.

Can we assume that from episode 7 on, Makoto is a woman?

She is just a reverse trap like any other.
And I don't know where you get that Wikipedia quote, because on actual article it describes exact opposite.

Judging by the fact that Makoto bandaging his chest, speaks of himself in a masculine gender and tries to fulfill a male social role, this is a transgender female-to-male.

I agree. Hope to see more of her background, even if it's not really needed, but that would explain her path.

Using the same logic, Mulan is trans, Irene Adler is trans and Maria Casterede is trans as well.
They are obviously not, but are just putting so much effort to be perceived as a member of opposite sex, despite still identifying themselves as their original one.
This is the very definition of a trap character and has nothing to do with LGBT community

But Makoto is an anime character that is positioned as intended for an adult audience. And his behavior is such that I can't identify him as a woman. And he's in love with Sawa, in a very strange way, but in love. I don’t mean to say that no exist lesbians or transgender people who prefer people of their true gender, but Makoto is not one of them.

If Makoto is really a woman disguised as a man, I am even more confused about the fact that the doctor said: „are you still wearing those implants?“ (or something like that) so what did that mean?

I think you misunderstood what the doctor said. He was talking about "prosthestic" in reference to Makoto's missing arm, which was lost during the fight against Janome on episode 4. He wasn't happy that she was so quick to use it because I guess it takes time to get used to it.

Well it was clear like the sky ..From the episode three she was revealed as a woman. Jin just slashed her clothes as a warning that nobody could hide anything from him. I too thought that she was a he from the first episode that I personally called him a handsome man in my first episode writing alas I didn't need to change the writing lol XD just need to accept the fact and move on haha
