Next episode also seem to be 1 episode short case, according to review.

And I thought in Japan they have mandatory health insurance? Maybe it was different in the 90ties?

/Edit: Now I entered 国民皆保険制度 歴史 in a search engine.
Result: There was general health insurance coverage in the 90ties, since 1961 in fact. The conditions were even better before 2003 than today.

If the mother of that woman is older than 70, the treatment cost would be fully covered by insurance by default.

Generally, the health insurance system in Japan makes those repeated cases in Kindaichi where people commit crimes to cover for operation costs kinda pointless. I was already wondering about that matter at a previous episode 99 where a father killed several people to get the funds to have his daughter treated.

For a while I was thinking Kindaichi was aiming to be somewhat realistic at least regarding the backgrounds, but apparently, it is not, since not even the basics of the social system are shown as they are. Well, I'll try to enjoy the show as it is.

/edit 2: Maybe Kindaichi depicts a parallel Japan without health insurance system. That could explain why there are so many crimes related to hospital bills in the series.
Kindaichi in a "realistic" Japan with its low probability of violent crime would be kinda boring I guess...
